Before Your Appointment

1. Call our office ahead of time if you would like us to verify your insurance benefits before your visit.

2. Gather any medical records, imaging reports, or relevant paperwork related to your condition.

1. A valid photo ID

2. Your insurance card(s)

3. Recent imaging reports and discs (if available)

4. Any previous medical records related to your condition (if available)

5. A list of your current medications

6. Any questions or concerns you’d like to discuss with the doctor

Self-Pay Patients Please Bring

1. A valid photo ID

2. Recent imaging reports and discs (if available)

3. Any previous medical records related to your condition (if available)

4. A list of your current medications

5. Any questions or concerns you’d like to discuss with the doctor

Auto Accident Patients Please Bring

1. A valid photo ID

2. Your insurance card(s) (if available)

3. Recent imaging reports and discs (if available)

4. Any previous medical records related to your condition (if available)

5. A list of your current medications

6. Police Crash Report (if available)

7. Photos of the accident, vehicle damage, or injuries (if available)

8. Emergency Room or Urgent Care records 

9. Auto insurance claim information

10. Attorney contact information (if applicable)

11. Any questions or concerns you’d like to discuss with the doctor

Work Injury Patients Please Bring

1. A valid photo ID

2. Your insurance card(s)

3. Recent imaging reports and discs (if available)

4. Any previous medical records related to your condition (if available)

5. A list of your current medications

6. Employer information

7. Workers’ compensation claim number (if available)

8. Workers’ compensation insurance carrier information

9. Adjuster or case manager contact information (if assigned)

10. Any work restrictions or medical records related to your injury

11. Any questions or concerns you’d like to discuss with the doctor

What to Wear

• Please wear comfortable clothing that allows you to move easily during your examination.

• For your comfort during your pre-adjustment massage, we recommend wearing a shirt with a plain back. Please avoid shirts with graphics, logos, raised lettering, or rubberized designs on the back, as these can interfere with the massager.  

If X-rays are clinically necessary, you may be asked to remove anything containing metal:

• Jewelry

• Belts

• Watches

• Eyeglasses

• Hair accessories containing metal

• Clothing with metal zippers or buttons

• Any other metal objects that could affect X-ray image quality

Please note:

• Depending on the area being evaluated, you may be asked to change into a gown.

• Our staff will provide clear instructions and make every effort to ensure you are comfortable throughout your visit.
